How to Refer

Please send referrals to:

Email: admin@consciousmindcentre.com.au
Medical Objects: Conscious Mind Centre
Phone: (07) 5616 2119

Once received, our team will contact the patient directly regarding availability, fees and next steps.

Medicare Mental Health Care Plans

We accept Mental Health Care Plan referrals through Better Access for patients with valid Medicare cards. We do not offer bulk billed services, patients will have out-of-pocket gaps fees after the Medicare rebates.

Please note that the Mental Health Treatment Plan itself is not the referral – a separate signed and dated referral letter is required.

To accept a referral, as per Medicare, the letter must include:

  • patient name, date of birth and address
  • diagnosis or presenting symptoms
  • reason for referral
  • current medications
  • number of sessions being referred
  • confirmation that a Mental Health Treatment Plan has been completed
  • referrer name, provider number and practice details
  • date and signature

The initial course of treatment can be for up to 6 sessions. Further referrals can be provided where clinically appropriate, up to the current Medicare annual limit of 10.

A new Mental Health Treatment Plan is not required each time a further course of sessions is referred.

You do not need to nominate a specific psychologist. If no clinician is named, we can allocate the referral based on presentation and availability.

WorkCover Queensland

We accept referrals for psychological treatment under WorkCover Queensland.

Please include where available:

  • diagnosis / psychological injury
  • date of injury
  • Work Capacity Certificate
  • claim number
  • case manager or insurer details
  • relevant work restrictions
  • treatment history
  • relevant reports

For psychological injury claims, early psychological treatment may sometimes commence while the claim is still being assessed, provided the required Work Capacity Certificate and claim requirements have been met. Our Administration Team can liaise with the insurer regarding approvals and treatment planning where required.

DVA

We accept DVA referrals for psychological treatment.

DVA psychology operates under the Allied Health Treatment Cycle. A treatment cycle provides up to 12 sessions or 12 months, whichever comes first.

Please include:

  • veteran name
  • DVA file number
  • Gold Card or White Card details
  • diagnosis / reason for referral
  • relevant clinical history
  • current medications where relevant
  • other treating professionals involved
  • referrer details and provider number

For White Card holders, please identify the accepted condition or relevant mental health entitlement where known. At the end of the treatment cycle, we provide an end-of-cycle report to the veteran’s referring GP with treatment progress and recommendations.
